• Be sure to bring in the following with your application(originals must be presented
• Child's Social Security card
• Child's Birth Certificate
• Wallet photo
• IEP (if applicable)
• Proof of medical insurance
• Current immunization records
• Most recent report card
• Parent Photo ID
• Two proofs of residency
